Community Care

Our Community Care team provide local care for local people in Lorne, Deans Marsh, Fairhaven, Moggs Creek, Aireys Inlet and Wye River.

We offer a range of programs and services that can be provided either on site or at your home.  Our goal is to support individuals and families with services across three specialist areas; Allied health, Community Nursing and Family/Community Programs.

Allied Health

The Allied Health Team offers a range of services and programs;

Community Nursing

The Lorne District Nursing service provides nursing care to the frail, aged and people with disabilities in their own home. The district nurse will also assist to coordinate services and supports to ensure remaining in your own home is as enjoyable and safe as possible. As part of our service we provide:

  • Clinical Assessment
  • Health education and information
  • Wound care
  • Carer support
  • Coordination of care needs
  • Advanced Care Planning
  • Respecting Patient Choices
  • Continence advice
  • Palliative Care

It is not necessary to have a referral to see the District Nurse. A client, carer or other family member can request a visit from the district nurse.

You may also receive a visit from the district nurse as part of a coordinated discharge plan after a stay in a public hospital.
